Fees

Individual Therapy Session (60 minutes): $200.00
Substance Abuse Evaluation (includes report): $650.00

Payment is due in full at the time of service. Sliding-scale fees available on a case-by-case basis. I accept cash and check.

Insurance

I am an in-network provider for Cigna and Horizon Blue Cross Blue Shield.

Note: If you are insured through another insurance company, you may be able to receive partial reimbursement for out-of-network services.  Consult your benefits schedule to find your plan’s out-of-network coverage.  You will be provided with a receipt to submit to your insurance provider’s claims department.

Cancellation and No-Show Policy

If you cannot make your appointment, please call at least 24 hours in advance to cancel or reschedule.  You will be charged the full fee for appointments missed with less than 24 hours notice.
